Service
This
service will give you the ability to distribute forms that
are mainly text orientated (except for say the logo) and
have been laid out to capture the maximum amount of data
but with user friendliness in mind. Additional parts of
this service include checking question layout for both clarity
and spelling and reverting back to you should any item be
unclear

Graphical
Service
This
option will give you the ability to distribute forms that contain
a greater degree of graphical content or that need colour content
included. The layout is usually designed to enhance the user friendliness,
clarity and professional image of the form and its contents but
may not capture the maximum amount of data in the space provided.
Additional
parts of this service include checking question layout for both
clarity and spelling and reverting back to you should any item
be unclear.
Pre
production proofs will also be provided so that you may make any
final adjustments as required.
Database
Creation
This
part of the service creates the type of job/task/survey to be undertaken,
e.g. Survey, Group Survey or Personalised Database as well as allowing
for data entry of details where necessary.
Survey
This
option is for surveys using an anonymous/non-personalised format
whereby forms are processed against a named database with the
ability to analyse individual forms or aggregate results.
Group
Survey
This
option allows users to generate a database containing details
of a particular Department/Schools/Faculty areas. Forms can then
be processed by these groups and the results stored accordingly.
Field details stored on the database include the group key and
description, both of which allow users to undertake structured
queries.
Personalised
Database
This
database type gives the ability to create individual records for
each participant, e.g. names, addresses etc., and then to undertake
structured queries on both database information and the responses
using the Access Database engine. The system will allow a maximum
of thirty user definable fields of up to 255 characters to be
defined on the screen and can include not only names and addresses
but also age, location, comments etc. to be entered.
Personalised
Database - Data Entry Manual
This
service allows for individual record details to be manually entered
into the system from printed lists supplied by the client. It
gives the facility to create separate records for each participant,
e.g. names, addresses, comments etc.
Personalized
Database - Data Entry Disk Transfer
This
method of data entry/transfer allows for vast amounts of individual
record details to be transferred from another computer system
to the Print & Scan system by either remote porting through
the Internet or floppy disk transfer.

Two
types of service are available for this aspect of the operation
as follows:-
Printing
OMR/ICR Scannable Forms (Masters Only)
This
service involves Trax UK in the production of the master form
templates only and is suitable for either standard or enhanced
surveys or personalised forms where bar code labels are to be
used on the forms. Once the masters are printed they may then
be reproduced through any good quality printing process (such
as photocopiers) by you. Additional parts of this service include
visually checking and test scanning the master forms through the
OMR/ICR scanners, checking the forms produced by you before distribution,
including alignment and test scanning and reverting back to you
should any problem be encountered.
Printing
OMR/ICR Scannable Forms (For Distribution)
This
option involves Trax UK in the production of all forms for distribution
to individual responders. The service includes visually checking
and test scanning random form samples through the OMR/ICR scanners
and distribution, including packing, labeling and posting to individual
addresses (stationary, specialised covering letters and postage
to be supplied by the client as required).
Printing Bar Code Labels
Trax
UK will produce Bar Code identification labels for personalised
forms. These may either be supplied to the client to affix to
the forms in the marked area or Trax UK can undertake this task.
Form Scanning
All
returned forms are scanned using optical mark reading (OMR) or
Intelligent Character Recognition (ICR) technologies. Past experience
has shown that the average scan rate varies due to some papers
being returned stapled, damaged, unreadable etc. and an additional
part of this service can include reprinting and marking all forms
not read during the scanning process.
Reporting/Results
Standard
Reporting Details
A
standard reporting service including printing aggregate totals,
both in results grid format and question format (either numeric,
percentile or both but by page only) is available. Structured
Query Formats (filtering) and Graphical reporting modes can also
be undertaken.
Export
Report Details to Disk
A
service to export the aggregate database details either over the
Internet or by floppy disk is also available as well as data transfer
of individual records to the same media.
Export Report Details to SPSS, Excel or Access.

This
service involves designing the HTML page that links to the Print
& Scan
Java Applet so that you may publish your forms to the World Wide
Web. Other services, such as form and database creation are as detailed
above.
